Agape Care Home is contracted with the Arizona Long Term Care System through UnitedHealthcare Community Plan, so eligible families can receive quality assisted living care without the full out-of-pocket cost.

Arizona’s Medicaid-funded program covering long-term care for seniors who qualify financially and medically. It can cover the majority of assisted living costs for residents who need daily support.
How we fit in
We’re an in-network assisted living provider with UnitedHealthcare’s ALTCS plan. Once a resident is approved, their care at Agape can be partially or fully covered depending on their plan and income.
