Care that fits your family's budget
Agape Care Home is contracted with the Arizona Long Term Care System (ALTCS) through UnitedHealthcare Community Plan, so eligible families can receive quality assisted living care without the full out-of-pocket cost.
What is ALTCS?
ALTCS is Arizona's Medicaid-funded program that covers long-term care for seniors who qualify financially and medically. It can cover the majority of assisted living costs for residents who need daily support.
How we fit in
We're an in-network assisted living provider with UnitedHealthcare's ALTCS plan. Once a resident is approved for ALTCS, their care at Agape can be partially or fully covered depending on their plan and income.
